Step 1: Download the "index.txt" file by clicking on the "DOWNLOAD FILE" button below ⬇
Step 2: Copy the code and paste it directly under [PART 1] in the "index.txt" file
Step 3: Copy the code and paste it directly under [PART 2] in the "index.txt" file
[Optional] Step 3A: The form provided only comes with text fields. Want to add more fields? Click HERE to view more types of input fields. It might require some knowledge in HTML.
Step 4: Save the file and rename the "index.txt" file to "index.html". Open the "index.html" file, you should see a form. Enjoy!
